  • Congratulations if your name has been drawn as a winner for one of my live stream, blog or Facebook giveaways! In order to receive the prize you have won, you will need to fill out this prize claim form with your name and mailing address.

    All winners are listed on the "Contests & Winners" page of my blog www.stampwithtami.com. If you are unsure, check the list.

    NOTES:
    * Sadly, currently I can only ship to US addresses.
    * Prizes must be claimed within 2 weeks (14 days) or they will be forfeited.
